I am an IRS Enrolled Agent and an authorized E-File provider. I have been preparing income taxes for individuals and businesses for the past 25 years.  I offer affordable, accurate and
professional tax services. I work with clients
nationwide to get refunds within 10-14 days.

• W-2s, 1099, 1098 and other tax documents sent to you by employers, government agencies, financial institutions and child care providers
• Copy of your 2010 tax return (for new clients)
  
• Completed Direct Deposit Form
  
• Voided check from account  for direct deposit (no deposit slips)
• Completed Personal Information Form
  
• Completed Rental Income and Expenses Form (if applicable)
  
• Completed Small Business Information Form (if applicable)
  
• Totals for sales receipts (do not send actual receipts)

• Read My 2012 Tax Letter for all Clients
• 2011 Personal Information Form
• Direct Deposit Form & Small Business Form
• Rental Property Summary Form

Rates

• $80 for most returns - includes long form 1040, Schedule A, NY State and E-file
• $60 for the short form (no schedule A, B, C, D or E)
• $10 for EZ returns of dependent children

1. Mail your return using US Postal Service only to:
      Ernest Palmer Tax Services, P.O. Box 384, Lewiston, NY 14092;
2. Drop your return at my house using drop box at the side of garage door;
3. Make an appointment to meet me at my office.  Please call in advance
      to set up an Appointment and use the side garage door.  No children
      please.  See instructions below for entering office.
